Pre-existing medical condition declaration forms
If you don't have any pre-existing medical conditions and you're under 81 years of age simply obtain an insurance quote with the option to then purchase your travel insurance online.
Please be aware of the definition of pre-existing medical conditions. If you have any pre-existing medical conditions please complete one of the following:
Fax the completed form directly to Cerberus Special Risks on 1300 613 193.
Once you have received confirmation from Cerberus Special Risks, obtain an insurance quote and read the different types of cover available, then purchase your travel insurance.
